Thank you for your enthusiastic response to the Health Assessment program.

To schedule an appointment, for an Occupational Health Screening, each firefighter must obtain an Information Packet from Kim Favorite or a Peer Fitness Trainer (PFT).  Link to PFT List below

Once you have a Packet you can contact Kat Yun by calling 206.781.6375 or e-mailing her at This email address is being protected from spam bots, you need Javascript enabled to view it to schedule the Health Assessment appointment. 

Due to the overwhelming response you may need to leave a message. When leaving a message, about making an appointment, there are three important points to remember:

• Do not leave the fire station number as a call back number.  Due to patient confidentiality, Kat will not call back fire station numbers.  Members should use their personal cell or home numbers.
• If your outgoing message does not include your name Kat cannot leave a message. Due to patient confidentiality if the outgoing voicemail message on your home or cell phone does not include your name Swedish can not leave a message. Therefore, if your outgoing message does not include your name, please provide Swedish with information as to what your actual voicemail message states so they know they have reached your specific voicemail box and can leave a message.
• Make sure you leave an e-mail address. This provides Kat a back-up method of contacting you. When providing your e-mail address: 1. State your e-mail address 2. Clearly spell out names or words in the first half of your address.

If you are having problems reaching Kat the "after hours" number (206.391.7634) is now also available during the day to take your calls. Please be prepared to provide the same three points, listed above, so that your information can be immediately forwarded on to Kat.

Members who leave a proper message can expect a return call within 24-48 hours.  If a message is left on Friday or over the weekend the call may not be returned until Tuesday morning due to the number of calls left over the weekend.

Appointments should be scheduled before the end of March to ensure that the Department can secure appointments through May 22^nd and/or request additional scheduling if necessary.

Once your appointment is scheduled you will receive an e-mail with the information you need to complete the appointment process.  This will ensure that you have received the most up-to-date information.  Samples of these documents you can go to have been provided on this site at http://iaff27.org/content/blogcategory/54/67/.
