Due to recent problems associated with members receiving billing statements for Grant Health Assessments related procedures administered by Swedish Health Services, a process has been put into place to facilitate any billing errors. Judy Goolsby has been assigned to handle any questions, concerns, and refunds related to billing errors. To date, most of these errors have been associated with the chest x-ray portion of the Assessment.
If you receive any billing that is associated with the Grant Health Assessments, please fax that billing directly to 206-320-2017. Please click here for a fax cover sheet to assist you in this process. For those that mistakenly paid a billing invoice, refunds are being processed.
